PAYMENT OPTIONS

Dr. Coonrad is covered as an out-of-network provider.  We do not participate in any insurance programs. We offer the following methods of payment. 

Please choose the option which is most effective for your budget.

Extremity adjustment …$45

• PAYMENT AT THE TIME OF SERVICE
  You agree to pay in full for your care on each visit.
  
  
• SHORT TERM PRE-PAYMENT PROGRAMS
  TEN VISIT PROGRAM: You agree to pay in advance for 10 visits and receive an additional visit at no charge. For example you can save $60 by paying in advance for 10 visits($600) and receive one additional bonus visit at no charge.  Additional fees may apply if extremity adjustments or myofascial release treatment is utilized in addition to chiropractic adjustments.*

         TWENTY VISIT PROGRAM:  You agree to purchase 20 visits in advance and recieve 2 additional visits and a re-evaluation ($120 value) at no charge.  A savings of $220.  Additional fees may apply if extremity adjustments or myofascial release treatment is utilized in addition to chiropractic adjustments.*

• EXTENDED CARE PRE-PAYMENT PROGRAMS
  Custom designed programs for you and/or your family are available that offer other larger discounts. These programs vary depending  upon the length of your specifically designed program of care.  Please ask for the details of these programs.*

           *Any refund from a prepaid program is reimbursed at the customary fee schedule.  Any unused portion of  a prepaid program is    written off  after one year of patient inactivity.

• INSURANCE ASSIGNMENT
   This program is only offered when multiple family members are receiving care at the same time or if your financial situation prevents you from receiving care. Once your deductible is paid we will bill your insurance company for your care and you pay the co-pay amount at each visit.
• MEDICARE  You agree to pay for care as you recieve it.  We will file your claim forms for you.
